設立理念

癌の内科的治療に関する学理の研究をもって人類福祉に貢献することを目的として本研究分野は1969年に設立された。（初め、抗酸菌病研究所臨床癌化学療法研究部門として設立される。）

歴史

斉藤達雄は、基礎研究部門として抗酸菌病研究所に新設された癌化学療法部門の初代教授に着任後、1969年（分野創設）に臨床癌化学療法部門の初代教授に着任した。斉藤は抗癌剤の臨床的評価を行うための効果判定基準を設け（小山・斉藤の効果判定基準）、米国で導入されていたフェーズスタディの概念に基づく評価を行った。様々な抗癌剤や多剤併用療法の臨床的評価を行ったが、当時固形癌では十分な臨床効果の得られ る治療法は開発されなかった。そこで1978年涌井昭が二代目教授に就任すると、抗癌剤の感受性、耐性、投与法といった抗癌剤の作用機序の基礎的研究にも力を注ぎ、新しい抗癌剤投与法の開発を行った。その結果、昇圧癌化学療法などの新しい投与法が開発され、臨床に応用されて、一部の症例に効果が認められた。1991年三代目教授金丸龍之介が就任した。1993年抗酸菌病研究所が加齢医学研究所へと改組され、本研究分野は腫瘍制御研究部門に癌化学療法研究分野として組み込まれた。1990年代以降の分子生物学の発展による癌研究の進歩により、癌を内科的に制御するためには抗癌剤の開発のみでなく、癌の生物学的研究が必要であると考えからがん遺伝子やがん抑制遺伝子に関する研究が推進された。2000年4月より研究所附属病院の統廃合に伴い、診療部門は医学部附属病院へ移行し、診療科名を腫瘍内科に変更した。2003年6月より四代目教授石岡千加史が就任し、がんの分子診断・分子治療法開発を研究テーマに、分子標的薬の臨床試験に加えて、遺伝子解析による予後や治療感受性予測を導入した臨床研究が行われている。さらにがん薬物療法に関するトランスレーショナルリサーチの実用化を目指した研究に取り組んでいる。現在の研究は癌化学療法に留まらず集学的治療を含めたより広い領域に及ぶため、2009年の分野開設40周年を記に、2010年から分野名を臨床腫瘍学分野に変更した。

研究テーマ
  1. 分子診断に基づく抗癌剤投与法の最適化に関する研究

    近年、癌の生物学的特性を遺伝子、ゲノムまたはトランスクリプトームレベルで解析することが可能になり、薬剤感受性（または耐性）に関する分子診断が可能になりつつある。我々は、マイクロアレイを基礎的・臨床的研究に導入し、ヒト腫瘍培養細胞株を用いた抗癌剤感受性遺伝子や発現プロフィールの同定や、腫瘍組織を用いた化学療法有効または無効群の発現プロフィールの解析に応用し、最適な抗癌剤投与法の開発を目指している。

  2. 分子診断による発癌リスク、原発不明癌、予後診断に関する研究

    我々は発癌リスクの評価研究として、これまで常染色体優性遺伝形式の家族性大腸腺腫症、遺伝性非腺腫症性大腸癌および家族性乳癌について分子診断システムの開発や、遺伝カウンセリング体制の整備に取り組んできた。今後は、大腸癌、乳癌に関して発癌リスク評価を行うためより簡便な分子診断法の開発を目指している。また、腫瘍内科では年間約20症例の原発不明進行癌の診療を行っているが、多くの場合、最適な化学療法を適用できず予後不良である。このような症例を的確に診断するための分子診断法を開発するために、アレイCGH法の導入やマイクロアレイ解析による癌種別発現プロフィール解析を行っている。さらに、消化器癌や乳癌の転移予測や予後予測を可能にする発現プロフィール解析と、その実用化に向けてより簡便で小規模な特定遺伝子発現プロフィール解析法の開発に取り組んでいる。

  3. 癌治療のための分子標的治療法開発に関する研究

    がん細胞に特異的な細胞増殖抑制やアポトーシス誘導を可能にする新たな分子経路を標的にする治療法開発を目指す。また、がん遺伝子産物の機能阻害や、がん抑制遺伝子産物の機能補填による分子標的治療法の開発を目指す。このため、より効率的なアッセイ系を開発することを目的にp53, BRCA1, PTEN, mismatch repair genesなどの機能スクリーニング系の開発に取り組んでいる。また、このようなスクリーニング系を用いて変異タンパク質の機能回復薬剤開発を目指したペプチド探索や共同研究により小分子化合物探索に取り組む。

  4. 消化器悪性腫瘍に関する癌化学療法の臨床試験研究

    我々の研究分野では、約40年間にわたり消化器悪性腫瘍（とくに食道癌、胃癌、大腸癌）の臨床試験に携わり、新規抗癌剤の安全性や多剤併用療法の有効性を明らかにしてきた。今後も、附属病院腫瘍内科を中心に、施設内または多施設共同研究で新しい癌化学療法の臨床研究に取り組む。

  5. ポジトロンCTによる癌の質的診断および治療効果判定に関する研究

    これは「機能画像情報医学に関する研究」という研究所のプロジェクトの一つであり、ポジトロンCTを用いて癌の質的診断、薬物療法効果の予測判定の可能性について検討をしている。また、最近次々と開発されつつある分子標的薬剤の適応・効果判定への応用の可能性の検討も始めている。

  6. 遺伝子操作によって作成した発癌モデルマウスを用いた発癌メカニズムの研究および化学発癌予防法の研究・開発

    大腸癌発癌に関与したAPCがん抑制遺伝子をジーンターゲティング法によって破壊した、大腸癌発癌モデルマウスを用い、様々な遺伝学的背景を要するアイソジェニックマウスとの交配と分子遺伝学的解析により、大腸癌発癌抑制および発癌予防に重要な標的分子を見い出し、これらを臨床的に制御する方法を開発する。
