Abstract The shortage of suitable transplant donor lungs remains a critical problem in the United States. The utilization rate of donor lungs is less than 20% with more than 300 transplant candidates dying or being removed from the wait-list due to their disease progression. A new innovative technology, ex vivo lung perfusion, has potential to solve this problem. Utilizing currently unsuitable for transplant donor lungs, ex vivo perfusion has the potential to repair these organs making them suitable for transplantation. A review of the current ex vivo lung perfusion research literature and clinical applications will be discussed.
